To be cleared medically by the athletic training staff, all student-athletes and parents of student-athletes (new, returning, and transfer student-athletes) must complete and return the following forms to the Athletic Training Department. All student-Athletes will have an athletic physical that will be conducted by the athletic training staff, the Cowell Wellness Center and team orthopedic physicians.  This exam will be arranged by the athletic training staff.  Physicals are mandatory for all student-athletes. 
Please note the informational packet mailed by the university is separate from the forms needed for an athletic physical.

Summer Physicals
If your student-athlete will be attending summer school at the University this year, you may complete your physical on one of the following dates:May 10, June 14 or July 19.  However, the following must be completed prior to attending summer physicals:  They must be enrolled as a student for the summer sessions, complete physical paperwork (including all laboratory work), and pay the health center fee.  Paperwork for summer physicals will be due by the following dates:

Fall Physicals
All teams will have an assigned date for fall physicals if you are unable to attend summer physicals.  You may contact your coach or Annette Martinez for these dates.  Physical paperwork for fall physicals is due by July 23.  Please send necessary paperwork for fall physicals to the following address.

Example Physical Day
7am-11am: Check-in and preliminary procedures done in the Athletic Training Room. Required for all athletes.

11am-4pm: General Medical Exams/Orthopedic Evaluations: the General Medical Exams will be done at Cowell Wellness Center (Each athlete will be given an appointment time to be seen at the Wellness Center).  Following the athletes general medical exam they MUST return to the training room.  Orthopedic Evaluations will be preformed initially by the Certified Athletic Training staff.  These evaluations will be performed for all incoming athletes and those with significant past/current injuries.  The Director of Athletic Training will determine participation clearance or restriction at this time. Required for all athletes.

5:15pm-6:30pm: Orthopedic evaluations will be done at the Main Gym Training Room.  The Director of Athletic Training will determine if a student-athlete needs to be seen by the Team Orthopedic Physician.

Please remember that NO physical activity (skill groups, weight lifting, testing, etc.) may be done before the athlete completes their physical and is cleared by the Director of Athletic Training for participation.
